Doula Support Services Request Form

We are honored to support you on your pregnancy, birth, and postpartum journey.

What is a Community-Based Doula?

A community-based doula is a trained birth companion who provides emotional, physical, and informational support throughout pregnancy, birth, and the early postpartum period. Unlike hospital-based doulas, our doulas come from the same communities they serve and offer culturally relevant care to ensure you feel empowered and supported every step of the way.

What to Expect from Your Doula

Once you complete this form, our team will carefully review your responses and match you with a community doula who aligns with your needs and preferences. You can expect to receive a match within one week.

Your doula will provide the following support, tailored to your individual needs:

- 4 Prenatal Visits - In-person or virtual meetings to discuss your birth plan, comfort measures, childbirth education, and any concerns.

- Labor & Birth Support - Continuous support at your home, birth center, or hospital during labor and birth.

- 4 Postpartum Visits - In-person or virtual check-ins to provide newborn care guidance, feeding support, emotional well-being check-ins, and resources for your recovery.

Your doula will walk with you through your pregnancy, birth, and postpartum journey, ensuring you feel supported and informed.
